The Registration Information Packet and Form are available
either together in a pdf format, or the form alone in a MS Word Form
format. The pdf file can be printed out
for future reference, or to print the registration form out to fill in by
hand. PLEASE READ THE INFORMATION IN
THE INFORMATION PACKET!!!! Once you
have read the information, you may open the Registration Form Only to electronically
fill out the form and either mail it to the registration coordinator and/or
print it out to mail. If you e-mail it
to the registration coordinator – you are not finished until you print it out
and send it along with your payment to the address on the form and listed
below. We must have payment to reserve
you a spot in the tournament!
The only thing we need to sign you up is the completed
registration form. PLEASE make sure all
emails are clearly written! This is how
you will get confirmation of your registration.
The forms listed below are to be completed once you’ve been
accepted. They are necessary for team
check-in, which can be completed in person the Friday before the tournament or
via mail prior to the Wednesday before the tournament. (See mail in
instructions)
